Please use only the current shoe request form that may be found on the “P” drive in the forms folder. Superintendent or designated representative should authorize issue of safety shoes. Note: There is no “annual replacement” category. Shoes are replaced when they are worn out. Worn shoes, being replaced, are to be turned in to Central Stores after you get your new shoes.
